The One Question That Matters Most
Exosome therapy works by delivering biological signalling molecules to the scalp that support the natural function of hair follicles. For that to produce a result, follicles need to be present and active in the area being treated.
If follicles are still present but miniaturising or temporarily dormant, there is something to work with. If they are gone, no regenerative treatment can restore them.
That single question, are the follicles still there?, is the foundation of any suitability assessment at Hair Revive, and it is answered most reliably by dermoscopy at consultation, not by what the area looks like to the naked eye.
The Bottom Line:
The earlier hair loss is addressed, the more options are available and the better the likely response to treatment. People who come in at the earliest signs of thinning are generally the best candidates. Those who wait until the loss is advanced have fewer options.
Who Is Most Suitable?
Androgenetic alopecia (pattern hair loss) in men and women is the primary indication for exosome therapy at Hair Revive.
This is the most common cause of hair loss, driven by the hormone DHT progressively miniaturising follicles over time. At early-to-mid stages where follicles are still active, it is the condition exosome therapy is most likely to help.
Traction alopecia caused by tension from tight hairstyles may be suitable if caught before scarring has occurred. At the non-scarring stage, follicles are stressed but still present. Once fibrosis has set in, they are not.
Telogen effluvium, diffuse shedding triggered by illness, surgery, childbirth or significant stress, may be suitable where recovery is slow or incomplete and follicles are temporarily dormant rather than lost. In many cases the priority is first identifying and addressing the underlying trigger.
Conditions That Are Not Suitable
Scarring alopecias including Central Centrifugal Cicatricial Alopecia (CCCA), lichen planopilaris and frontal fibrosing alopecia involve inflammatory destruction of follicles and replacement with scar tissue. Once scarring has occurred, there is nothing to regenerate. These conditions require specific dermatological management, not hair restoration treatment.
Active alopecia areata is an autoimmune condition requiring its own treatment pathway. Cases in sustained remission may be considered at consultation; active disease is not appropriate.
Advanced pattern hair loss where follicles in the target area are no longer functional is beyond the reach of any regenerative approach. Surgical options are the more appropriate conversation in that situation.
Other Factors
Exosome therapy is not recommended during pregnancy or breastfeeding. A known fish or salmon allergy is a contraindication to the E50 protocol. Active scalp infections or significant inflammatory skin conditions in the treatment area require resolution before treatment can begin. Patients on immunosuppressant therapy or active chemotherapy require a detailed medical review before any suitability decision.
